So was finally able to reproduce a nice side effect of setFillWindowForwardOnly and as confirmed 100% unrelated to that option 
Ineresting. So the issue is related to how android displays the list? Oddly the issue disappeared after toggling the provider setting, which prompted me to believe it to be the root cause.
It’s more complicated but it’s just coincidence that it did not happen anymore. Do it more it will still have possible duplicates.
